Employee Benefits Survey - Health Insurance

Revive recently conducted a survey of interest in various benefits that could be offered to Revive staff.  This survey is a continuation of the process, and will focus exclusively on HEALTH INSURANCE.

Please complete the following survey, so that Revive can continue to evaluate the viability of offering additional benefits to staff.

DISCLAIMER: This survey is exploratory in nature.  The goal is to collect information that can be used by Revive to make informed long term decisions about the needs and desires of its staff.  Revive is not committed to (or even leaning towards) implementing any of the proposals below.  Revive staff should not take any part of this survey as an indication or a promise of changes to future benefits that are offered by Revive.

PARTICIPATION REQUIREMENTS

Please assume the following restrictions and requirements when answering this survey.  These requirements are NOT definite, and could change based on a variety of factors.

All Employment Types:

  • There will be a 90 day wait period from the start of Employment period to the start of the Qualifying period.

W2/Employees:

  • Must work an average of 20 hours per week in a salaried or hourly position.
  • Coverage would stay in force for qualified time off (Short/Long Term Disability, PFML, maternity Leave, and similar).

1099/Contractors:

  • Must average 40 billable sessions per pay period (20 sessions per week) in at least 2 of the previous 4 pay periods (4 of the previous 8 weeks).
  • Coverage would stay in force for qualified time off (Short/Long Term Disability, PFML, maternity Leave, and similar), provided that the contractor continues to make payment on their portion of the premium payments.
